Introduction

Empire State Wellness Inc. ("we," "us," or "our") is committed to transparency in our fundraising efforts to support dental grants for veterans and first responders in New York. This Fundraising Disclosure provides detailed information about our solicitation practices, the use of donated funds, and compliance with applicable laws.

Fundraising Practices

We engage in fundraising activities to support our mission of providing dental care to those in need. Our methods include:

  • Online donations through our website (https://empirestatewellness.org/donate/) via Donorbox integrated with Stripe.
  • Mail-in contributions, with instructions available on our Contact page.
  • Potential future campaigns on social media platforms and community events as we grow.

All fundraising is conducted by our staff or authorized volunteers, and we do not currently employ professional fundraisers.

Use of Donated Funds

Your contributions are vital to our programs. We allocate funds as follows:

  • The majority of donations support direct dental treatments, such as implants and full-mouth reconstructions, for approved grant recipients.
  • A portion covers operational expenses, including website maintenance, administrative costs, and compliance efforts, to sustain our nonprofit activities.
  • We prioritize transparency and will provide periodic updates on fund allocation as our financial data becomes available.

Tax-Deductibility and Legal Status

Please note the following regarding your donations:

  • Our 501(c)(3) status is pending with the Internal Revenue Service (IRS). During this period, donations are considered tax-deductible under interim IRS guidelines for organizations applying for tax-exempt status.
  • Tax receipts will be issued via email upon donation, but you should consult a tax professional to confirm your eligibility for deductions.
  • Once our 501(c)(3) status is approved, we will update this disclosure and notify donors.

Compliance with New York Law

Empire State Wellness Inc. complies with New York Executive Law § 172, which requires charities soliciting over $25,000 annually to register with the New York Attorney General’s Charities Bureau. As a new organization, we are below this threshold currently but are in the process of registration. Once our annual contributions exceed $25,000, we will file the required CHAR500 form and make financial reports available as mandated.
