Your donation is intended to support our mission of restoring smiles and dignity through dental care. Please note the following:

• Empire State Wellness Inc. is a Delaware nonprofit corporation recognized by the Internal Revenue Service as a tax-exempt public charity under Section 501(c)(3) of the Internal Revenue Code (EIN 39-2099691). Our exemption is effective May 8, 2025, per the IRS determination letter dated September 25, 2025. Contributions are tax-deductible to the extent allowed by law. We provide a tax receipt via email. Donations processed through our third-party provider (Donorbox integrated with Stripe) are irrevocable. Refunds will only be issued in exceptional circumstances (e.g., processing errors) at our sole discretion or as required by law.

Empire State Wellness Inc. is registered as a nonprofit in New York, with ongoing registration with the New York Attorney General’s Charities Bureau. We comply with all applicable federal and state regulations, including those governing charitable solicitations.
